Steps to Become a Medical Marijuana Patient in Michigan

Step 1: Complete the Michigan Medical Marihunana Program (MMMP) Application Packet (First Icon Below | Page 1)

Step 2: Obtain a signature from a physician (either an M.D. or a D.O.) attesting to the State that you will benefit from the use of medical marihuana. It is up to you to provide independent evidence that you indeed do have one of the following conditions: Cancer, Glaucoma, HOV/AIDS, Hepatitis C, Lou Gehrig’s disease (Amyotrophic lateral sclerosis, or ALS), Crohn’s disease, Alzheimer’s, Nail-patella syndrome Cachexia (wasting disease), Characteristic of Epilepsy, Severe and Chronic Pain and Severe Nausea, Seizures, Muscle spasms, or Multiple sclerosis. (First Icon Below | Page 2)

Step 3: The State requires a $60.00 check or money order to accompany the application packet. Make checks or money orders payable to: State of Michigan-MMMPAdditionally, if you have a caregiver, you must provide a $25.00 fee for their background check. Include a copy of a valid photo identification (Example – Michigan Driver’s license or Michigan ID card). All items listed above are then sent to the State.
